Paxton Victoria Hazlett

Paxton Victoria Hazlett, infant daughter of Robert Hazlett and Rickelle Foraker, died Sunday (November 6, 2011) at Nationwide Children's Hospital, Columbus. She was born November 2, 2011 in Columbus. She was preceded in death by a great-grandfather, Frederick Burgy; a great-aunt, Margaret Anderson; and two cousins, Melissa and Melody Anderson. She is survived by her mother and father, Rickelle Foraker and Robert Hazlett; a brother, Kason Hazlett of the home; maternal grandparents, Jason & Lauren Foraker of Cambridge and Kari & Patrick Bintz of Zanesville; paternal grandparents, Brian Hazlett of Columbus and Tabitha Neff of Zanesville; maternal great-grandparents, Glenn & Rita Foraker of Cambridge and Anna Burgy of Cumberland; and paternal great-grandparents, David & Jackie Morrow of Roseville. Family and friends may call on Thursday (November 10, 2011) from 11 a.m. to 1 p.m. at Thorn-Black Funeral Home, Cambridge, where funeral services will follow at 1 p.m. with Evang. Mark Beros officiating. Burial will be at Friends Cemetery, Quaker City.
